| PHYSICAL EXAM REQUIREMENTS: |
|
Grade 7 Physical Exam Requirements A Physical Exam is required for every student prior to entry into grade seven. You MUST have your doctor's office complete a State of Connecticut Department of Education Health Assessment Record. These can be obtained from any school's main office.

Grade 8 Physical Exam Requirements Sports Participation Health Record Any student who will be TRYING out for sports (Interscholastic or Unified Sports) will need a physical examination, dated within the past year, prior to the tryout.
FREE SPORTS PHYSICALS are offered for incoming 8th graders in August before the start of school by the School Physicians, Dr. James O’Connor and Dr. Karoline Kinsella. The dates will be posted on the Dodd Calendar of Events which can be accessed by clicking on the link above. These physicals will be good for the entire school year. Students who missed the August sports physical and decide to try out for winter or spring sports, should contact the nurse for information on getting a free sports physical at CHS.

| IMMUNIZATION REQUIREMENTS: |

Grade 7 Immunization Requirements The following immunizations are required prior to entry into grade seven: > 2nd MMR > 1st in a series of 3 hepatitis B shots > Doctor's proof of Varicella(chicken pox) Disease or the Varicella Vaccine

Grade 8 Immunization Requirements Completion of the Hepatitis B series is required BEFORE entry into 8th grade.

| PRESCRIPTION AND OVER THE COUNTER MEDICATIONS: |
|
ALL Medications given in school or on field trips MUST have a Doctor's signature. This includes over the counter medications, i.e. Tylenol, advil, motrin, decongestants etc.
Once medication authorization forms are filled out, they will be good for the entire school year. If there are medication changes or additions, a new form must be completed.
Separate forms are required for the Washington D.C. field trip. These medication forms can be found on the Washington D.C. Trip Web Page (link below). |
